A person-centered approach for quality supported living services.

For adults with disabilities who need support to live independently from a few hours a day to 24-hour care, we offer residential placement in a comfortable home setting tailored to meet the needs of each client.

We encourage each person to use their voice in directing their own supports. We work collaboratively as a team with each client, their families, and any professionals involved in their care. Starting with a CARE assessment and Person-Centered Planning, our staff develop an Individual Instruction Support Plan (IISP) that identifies needs and goals while providing a path for clients to follow in pursuit of gaining independence and expanding personal enrichment.

Clients live independently and learn to make connections with others. Their individual preferences for celebrating holidays, participating in planned activities, and going on trips with their friends is encouraged. There is much fun to be had meeting new people, gaining new skills, having new experiences, and participating in their community.
