A fast track to your ideal job.

Are you having challenges finding or keeping a job? Our experienced professionals have the passion and skills to assist individuals 16 and older with complex health needs, mental health diagnosis, substance use disorders, and disabilities who experience challenges with finding and retaining employment. Services are delivered through Foundational Community Supports, a person-centered and evidence based practice. We will find you a job aligned with your preferences in your community and provide support to help you maintain and grow in your job and career once you are employed.

You will not be excluded from service if you have challenges to employment such as current symptoms of a mental health condition, unsuccessful work history, criminal history, lack of training or experience. We target regular jobs in the community which offer you increased self-sufficiency, status, and career development potential.

Appropriate for:

  • Participants must be eligible for Medicaid
  • Individuals enrolled in state Housing and Essential Needs, or Aged, Blind, or Disabled Program
  • Individuals with mental illness resulting in the need for improvement, stabilization or prevention of deterioration of functioning

  • Individuals diagnosed with a substance use disorder

  • Individuals who need assistance with three or more activities of daily living
